Submit a Claim - Claim Instructions

If you received a prerecorded voice call to your cell phone and the call was transferred to a PillPack call center as part of the PillPack Performance Media campaign, you may be entitled to a payment.

If you are a member of the Settlement Class and wish to receive a payment under the Settlement, you must submit a valid Claim Form so that it is postmarked (if mailed) or submitted online no later than January 20, 2025.


How to File Online:

In order to file a claim online, you will need a Claim ID or the phone number at which you received telephone calls from PillPack LLC. Your Claim ID is located on the Notice that you received via mail or email. If you lost or deleted your Notice, please contact the Settlement Administrator at 1-855-654-0837.

After submitting your completed Claim Form online, you will receive an email with a confirmation code for your completed submission. Receipt of a confirmation code means your Claim Form was successfully submitted. If additional information is required to complete your claim, you will be contacted by the Settlement Administrator.



How to File by Mail:

Fill out, sign, and date the Claim Form, then mail the Claim Form to the following address:

To be effective as a claim under the proposed Settlement, this form must be submitted or postmarked, as outlined above, by no later than January 20, 2025.
